Attention Student-Athletes and Parents of Student-Athletes
The Michigan High School Athletic Association (M.H.S.A.A.) requires each student-athlete to have a valid physical on file with the school before they may participate in Middle School or High School athletics. A valid physical for the 2018-19 school year is one that is given on or after April 14th of2018.
NEW IN 2017-18: The M.H.S.A.A. will now require a NEW sport's physical form that has concussion acknowledgement information. Please make sure you use the following formM.H.S.A.A. Physical Form. All other forms will be invalid.
Hometown Urgent Care is no longer offering the "Give Back Program" like they did in the past. However, they will still be offering low cost physicals for a $25 fee. Hometown Urgent Care has locations on Gull Road, Westnedge Ave. and Centre St. in Portage.
