UPDATE: The M.H.S.A.A. is relaxing the athletic physical requirements for the 2020-21 school year. The information below may not be relevant for any student-athlete who had a valid physical on file during the 2019-20 school year. Please note, each student-athlete who ALREADY has a valid physical on file will need to complete the MHSAA Health Questionnaire. Here is the link.
The Michigan High School Athletic Association (M.H.S.A.A.) requires each student-athlete to have a valid physical on file with the school before they may participate in Middle School or High School athletics. A valid physical for the 2019-20 school year is one that is given on or after April 15th of 2020.
The M.H.S.A.A. will now require a NEW sport's physical form that has concussion acknowledgement information. Please make sure you use this form for the 2020-21 school year. The other form will be invalid. M.H.S.A.A. Physical Form
